The Field Medical Card is usually initiated by the combat medic. However, the
combat lifesaver can initiate the Field Medical Card if no combat medic is available or if
the combat medic directs the combat lifesaver to initiate the card. A pad of Field
Medical Cards is part of the combat lifesaver medical equipment set.
a. Remove Protective Sheet. When initiating the Field Medical Card, remove
the protective sheet between the original card and the duplicate (white sheet). The
back of the original card is impregnated so that the information written on the front of the
card will also appear on the front of the duplicate sheet.
b. Enter Primary Information. Complete blocks 1, 3, 4, 9, and 11, if possible.
c. Enter Secondary Information. Complete blocks 2, 5, 6, 7, 8, 10, 12, 13, 14,
15, 16, and 17, if appropriate, as time permits.

PRIMARY INFORMATION
a. Block 1. An illustration of block 1 of the Field Medical Card is shown in
figure 7-3. Information may be obtained by talking to the casualty, from the casualty's
identification tags or uniform, or from other soldiers.

Name. Enter the casualty's name in the box titled "LAST NAME, FIRST
NAME/NOM ET PRENOM." Enter it in last name, first name, middle initial format.
(2) Rank. Enter the abbreviation of the casualty's rank (SGT, CPT, and so
forth) in the box titled " RANK/GRADE."
(3)
SSN. Enter the casualty's social security number in the box titled
"SSN/NUMERO MATRICULE."
(a) If the casualty is a member of a foreign military, including a prisoner
of war, enter the casualty's military service number instead.
(b) If the casualty is not military, leave the block blank.
